Congregations — Download a blank PDF statistical report

Congregations may download a blank PDF statistical report form on the LCMS Information Support Network (LISN) website.

If you do not know your congregation’s login information, an authorized staff member should contact the LCMS Church Information Center.

To request a blank copy, an authorized staff member should contact the LCMS Rosters, Statistics, and Research Services.
